Dear Parents and Guardians,
As we prepare for a healthy and successful start to the new school year, please remember to submit your child’s school physical examination and required immunization records as soon as possible.
Up-to-date health records are required for school attendance. If your child is entering a grade that requires updated health forms or immunizations, please turn them in to the school as soon as possible, all must be submitted by Friday, August 14th. You may also bring the required forms, medications and completed medication authorization forms to your child’s open house.
